Maybe you could support his head in his car seat with cushions, then put a cute toy in his lap. Does he have a physical therapist? They might have the equipment you need. Their stuff is usually plain black but you can cover it with some fabric to match the background you choose. You might want to go see the backgrounds your studio has available (for instance if you use an outdoor/park background, you might get a piece of grass green fabric and drape it under him, and over the seat, to make his support blend in.
I think you will want the picture to show his sweet face mostly, so don't let the fixings take away from that.

Is there any way you could get a photographer to come to your house? That way he could be in a chair or even his bed with stuffed animals and things? Or even outside laying in the grass would be very cute. Or you could hold him in your lap for a few of the shots. On his tummy if you could get his arms under his head like he was resting it there would be another option.

A photo of him sleeping peacefully or even awake on some nice fluffy cloud like material with a blue sky background would be lovely. Have him dressed in just his nappy, with angel wings laying on his back, and a gold tinsel halo on his head.

Ive seen cute ones of kids lying on their backs and the photographer takes it from above. A lot are out in grass. Also seen them where the kids are sitting in washtubs or baskets. Id just do some searches on "childrens photography" or babies, and look at the photographers galleries for ideas too.

Get a large teddy bear that is positioned in a sitting position and prop him against it so that it looks like the teddy bear is hugging him. that way he will be sitting up and less likely to fall over. Make sure the teddy has sturdy arms and legs.

Do you have any other childen, maybe older? Or your husband? Or you? You can lie down and put your son on your back and hold onto him. Have him put his head on your head, and have your partner lean on your side so he can roll hhis head to one side if needed. Otherwise, you could put a toy/teddy in front and have him put his head on that.
