it depends. sometimes it is the bridesmaids. all the ladies at my church threw me one. if you work at a cool place with nice people, your co-workers could. :)

Usually the maid of honor and/ or bridesmaids. Most of my wedding party were from out of the area though so my mom planned it and then gave each bridesmaid something to be responsible for during the party such as different games or activities so everyone chipped in, but it was easier for mom to coordinate being that it was at her house and local.

I think is no ones responsibility is not really necessary to have a shower for the bride, but if they want to organize one for her i think it should be her sister/s or her best friend/s.
