Hmm, well . . . I worked in a nursing home for a couple of years once upon a time. I never witnessed this. I'm not saying it's impossible, just never saw any, shall we say, "evidence" of this happening.

An orgasm is an explosive discharge of neuromuscular tensions at the peak of sexual response. An orgasm usually happens with sexual stimulation. That would be our basis to consider the impossibility of an orgasm prior to death. There is no any evidence to prove that but it is odd to think that a dying person could have some sort of sexual stimulation to attain an orgasm.
Though some sexologist had mentioned that sexual orgasm and dying share some key physiological similarities- not the least of it is the occasional loss of consciousness. It is said that some (and perhaps most persons) may become momentarily unconscious at the moment of orgasm, and some may remain unconscious or only vaguely aware of reality throughout the spasms or convulsions which follow orgasm similar to epileptic fits and physiologic responses to electric shock. Such scenario in comparison can be also observed among the dying. Though it could invite some confusion of what really is taking place on the dying person manifesting an "orgasmic clues" it is hard to consider that there is any sexual stimulation right from somewhere to doubt of it as an orgasm.
I witnessed the above scenario described prior to death of dozens of persons. Personally, it would be hard for me to believe that there is such an orgasm prior to death. An experience similar to orgasm could probably take place but it is far beyond sexual stimulation. But anyway, who knows?

imho The actual dying would not cause orgasm. However sadistic or masochistic imaginations could cause an orgasm in such instances as:
• standing naked before a firing squad as the order to fire is given
• being shot with arrows as in a sacrificial rite or as an amazon warrior.
• being hanged or autoerotic strangulation.
I sometimes wonder if some suicides are not prompted by these imaginations.
There are several fantasy websites. usually involving women,. that convey that fantasy.

Seriously, it is possible. But not for the reasons that you think! Just before death, most of your organs and senses would fail. Consequently, you might have unusual dreams, pleasant or unpleasnant odors, the feeling of flying, tears, intense love or hate, you might urinate, dirty your pants and possibly orgasm. But all these things will not happen to everybody all at one time.
