Actually, they did use middle names then, but they used it differently. Your middle name was your fathers name.
He wasn't known as Jesus Joseph (which is how the Bible traces his geneology, surprisingly enough).
He was known as Jesus of galilee (which was a common type of nick name) and Jesus son of Mary.
