All the above advices are OK. But heres another

Give the same camera to 2 different people and you can get one person to take "clear" picture while the next person may always get blurry pictures.
It is just a bad habit started by people who misunderstand instant (Point and Shoot) cameras. By this what I mean is that most people will count to 3 and hit the shutter on 3.
THEN start moving the camera on 3.1 (split second after hitting the shutter). The truth is that often your camera is NOT done taking the picture until the count of about 3.5 of even 4.
So next time try count
1 (shutter half way down to focus),
2,
3 (shutter and hold),
4,
5 (relax).
And hold the camera absolutely steady till 5. And don't stab the shutter but gently and slowly squeeze the shutter so the camera does not move during exposure. Practice by your self. And continue to practice until it becomes second nature.

My wife just got a iPhone. The built in camera is kind of slow. Yet when she takes the picture the pictures always comes out clear. But when her friends takes the picture, ALL of them comes out blurry. And the same people always have the same blurry effect. That's because the camera is so slow the user needs to hold it very steady for few seconds.

Your lens might be dirty or your hands shake a bit when you press the button. Also, sometimes with digital cameras using the auto focus feature is not always the best option. Here is why. In dim lighting conditions, the camera has serious problems trying to focus the lens properly. It needs to see clearly so it can focus the image prior to the photo being taken. The same is true with the human eye as well. Other times, your subject might be standing either in front of or behind a larger object such as a road sign or billboard. The camera then may focus on the sign instead of your subject. When that happens your subjects face is blurred a bit but the background is clear.
Best thing to do when that happens, is to have your subject stand NEXT to the sign rather then in front or behind it.

Clean the lens. Focus if you can. Hold the camera steady. Increase the pixel count.

Well, its a camera phone so they aren't going to be top notch quality. Make sure you hold your phone very still and don't move it until you're positive the picture has taken.
