At some point in a future conflict will the losing side turn to nukes in order to try to win ?

All land based weapons MUST BE LAUNCHED if a massive strike is detected at launch.
Sea launched weapons will be held in reserve for follow on strategic retaliation AFTER evaluation for follow on targets.
Since the fixed launchers are most vulnerable for first strike destruction they need to be UIOLI targeted. (Use It Or Lose It).
One problem is if you use ALL of your land based missiles to target Russia and you have not launched against China because they have not launched against you, then you must have a follow on strike capability.
Other land based weapons are held in reserve that are not based in CONUS. These and most of the nuclear strike capability on submarines should be prudently held in reserve for follow on targeting should China decide that it is a good time to also join the game.

I believe that we should make it known to Iran and other such nations, that in the event of a terrorist nuclear strike on the US or its allies that there will be an immediate and massive nuclear strike on their capital cities.
This may sound cold and cruel but you do not reason with terrorist, mad dogs, or the demon possessed intent on your death and destruction.

Early detection does not prevent a nuclear strike. It only gives you early warning that they are on the way.
Get all of your bomber strike forces up ASAP.
Adjust your missile targeting based on where the launch country is.
Launch all land based missiles if they are being targeted.
Initiate preplanned orders to all forces.
Alert Civil defense.

No nuclear war is not inevitable by definition but still remains an almost-likely possibility. The biggest threat is from proliferation. The best hope is to see that any new countries not join the "Nuclear Club". A limited exchange could cause a global chain reaction drawing nations into a global nuclear war. That of course is the worst case scenario. It seems definitely that somewhere, sometime a nuke will be used militarily again.

Maybe not all out nuclear but there is the possibility of a small scale nuclear conflict. Remember a couple of years ago, when India and Pakistan almost went to war? Both are nuclear armed and a major war there could end badly with one side deciding to teach the other a lesson.

Not necessarily, clear minds prevail. Let's hope it stays that way. A nuclear war affects all, even the aggressor, so it probably won't happen. Imagine this scenario: Russia shoots off their nuclear weapons and in response, 30 seconds later we fire ours, then theirs hits and blows us out of the water, then ours hits them and blows them out of the water. Radiation is wafted on the jet stream from both devastated continents and wipes out another 25% of the world, the rest of the world with or without nuclear capabilities panic and start volleying their own weaponry what have me got but mass annihilation. Let's jus hope nobody gets trigger happy.

IF Pakistan falls and Musharraf is killed or deposed, yes it is inevitable.
If Iran obtains nuclear weapons, yes it is inevitable
If the US politicians grow a spine and disarm these countries that either support terrorism or have lost control of their borders and people then it won't happen.
If I had to bet the first strike will be Israel.

I believe so. The human race is growing provably stupider. There's got to come a moment where some maniac somewhere decides to nuke the infidels, or fulfill the holy rapture, or some such creepiness, and once a couple of those units go off, as any intelligent general will tell you, every one one of them will get used.
