What would you say, how would you react, and most importantly, how would you avoid embarassment?

If it wasn't a lot, I would probably just sit there, and say nothing about it. I would wait until everyone left, then leave and go home. If it was a lot and people noticed, I have no idea. I would probably just excuse myself and leave very quickly. Then never go back!

if you have problems with incontinence, I'd advise going to see your doctor - there is a possibility that there is a simple cure for your particular condition - or a method for limiting the embarrassment.

An answerer above has mentioned pre-planning by having a plan or action - include your teacher in this - perhaps if you were to indicate with a discrete signal, he or she would know you are in trouble and could do something to help you out of this potentially embarrassing situation. If the worse were to happen, you would need to make your teacher aware of the situation in any case.

Have a plan of action in advance. You don't indicate if you know in advance of the 'urge'; if not-you might want to wear 'adult diapers'. You may want to have a friend in class that you have a code signal you can use to indicate that you need to leave-NOW-and have them assist you with a speedy departure.
In reality, the wheelchair is not really the issue, how would your respond if it weren't for the wheelchair? Respond the same way.
