Jumping out of gear manual trans means some bearings in the gear box has failed and there is to much end play causing the selector problems staying in gear. Sycronizers selector rings them self wear and may cause jumping out of one gear.Generally these are also the extra noise you hear when you let the clutch out in neutral. In very rare cases the shift linkage comes loose from the transmission you feel that by lifiting up on the shifter knob... Lots of up and down movement? Automatic transmission neutralizing can be causes by clogged screen on the oil pick up to the pump.

The dog teeth on the syncronising rings in the box are rounding off/wearing out, either that or the selector detents are weak and not holding the selector rods in place.
Either way your gearbox has to come out and be stripped to fix the problem.
