There is no way to know this without a physician looking at it. Some sexually transmitted diseases such as Syphilis initally cause lesions (called Chancre) on the penis, anus or on the vagina. The chancre is usually painless and heals on it's on- but the disease remains in the person's bloodstream and moves on to the second stage of infection. My advise is to see a physician, as he or she can perform tests to determine if this is just a pimple, boil or some type of disease process.

A small pimple on the buttocks may be nothing more than that, an infected oil gland or hair follicle. A larger pimple, more like a boil, could be the sign of a fistula in ano or possibly a peri-rectal abscess, especially if located very near the anal opening. The latter two are tender or downright painful.

Well, there is the chance that it could be genital warts that have spread to that area!! It is quite uncommon to get a "pimple" in that spot!!! There is the outside chance that it could be an ingrown hair, especially, if that area is shaved.

It's most likely due to perspiration and the male needs to wash more often to keep clean.
