Yes, but it's rare. They usually watch that and give fluids to prevent it.

Secondly, a heart attack is when you don't get bloodflow to a part of your heart for some reason, and since there is no blood, there is no oxygen and the tissue is damaged and/or dies.
Yes, someone can have a heart attack on the table if they lose enough blood. When you lose blood, your blood pressure drops. Your heart is selfish and takes as much oxygen out of the blood as it can for itself, but if there isn't enough blood to carry it, or not enough blood to pump through the body, then yes, a heart attack can be the result.
A person will become tachycardic initially when they lose blood because their body is trying to compensate. It says 'ok, not enough oxygen, if i pump faster, maybe I can get more' But the faster it pumps, the more oxygen it eats, and if there isn't enough to begin with, it's making the situation even worse.

In theory yes, although that isn't a lot during surgery when you are being watched. tachycardia is the heart beating rapidly and doing little movement of blood, heart attack is normally something else that does damage.

your question is ambigiouos, but bradycardia is usually associated with blood loss heart attacks...initially there is tachycardia to compensate the blood loss but as more blood is lost the heart has less nourishment and losses power
