Yes. When my husband has been hospitalized for "other" heart issues, he's attached to a telemetry unit that monitors his heart activity - the nurses always come running when he goes to the bathroom - having a bowel movement is actually hard on your heart and lungs.

You can have a heart attack at any time. I'm assuming though your referring to a vaso-vagal response. When pushing to have a bowel movement blood pressure temporarily drops and the person passes out briefly.
