First of all you should consult your consultant, & see his recommendations.
If it's OK, ask an airline to provide you a data of facilities provided by them, recheck this facility with your consultant.

I think break journey will do for long term travel.

Ask your doctor, if the airline knew they probably wouldn't take you due to liabilities.
I couldn't even go by vehicle without lots of stops for many weeks.
