Presuming that your card had not yet expired, you can ask for card replacement from the place who issued you such card. You may be required to pay $10.00 or so. Taking along with you the BLS certificate would hasten the process, however if you do not have the certificate as well, at least you need to recall the date you had the course. Anyway, with the computer system we have now, your name will be at the database so tracking the register down won't be that difficult.

Yupperlicious, just like the dude in the front of me, ask for a new one nicely, but dont take the course over again, its expensive... and besides, you are trained already, so technically, the card is just there if the the person you are treating doubts you, (which may result in their own death) so you don't need it.

Contact them for a re-issue, or just take the class over again.
