I am sure you can get them from any medical supply store, but also there is a software program on line that you can enter your blood pressure readings and it will chart it for you etc. (see blow link). Or you can just type them up with dates and times, etc.

The easiest way to do that is print out the chars yourself. Type them up and print them.

Why would you need one? It would be easy to produce one on your computer anyway.
