it would depend on a lot of factors including the following..
* Any symptoms of heart failure/ any heart failure medicines that he is on?
* Location of the hole (ASD or VSD)
* Whether his growth is affected by the hole or not
* Is the hole growing smaller in serial ECHOs
As a general rule most ASDs should be operated at this age, and if the VSD is causing any symptoms / is not getting smaller then this should be operated too.
For ASD it can be done without a surgery via a percutaneous guidewire (like angioplasty) and do a device clsoure .
Nowadays some centres are even not required to operate for certain special types of VSD too, they can try with a device closure for it,

The answer to that question depends on many things, such as where the hole is, what effect it's having on the circulation, and what condition the patient is in. The only person who's in a good position to answer these questions is a pediatric cardiologist who has had a chance to examine your neice's son, there is simply not enough information here to make any sort of determination.

Talk to the cardiologist. I can tell you I work at a children's hospital and it is not uncommon to operate on infants much younger than your nephew. Most are out of the PICU within days of surgery.

It would depend on how badly it is affecting his lifestyle. Repair is a very safe operation and personally I would go for a repair sooner rather than later.

Consult a heart specialist and do what he says. There is no medicine for congenital disease. Only surgery is the solution. Where are you located? There are good hospitals in Tamilnadu and Karnataka.
