Same deal as your Federal and State Income taxes. Tax is withheld during the year but you settle up at filing time.

I don't know why it seems like you're paying it twice. It's the same as federal income tax - at the end of the year you compute the total you owe, and subtract what you already paid in - if you owe more, you pay the difference, if you paid in too much, you get the extra refunded.
