Upper RIGHT back pain, severe shoulder pain (it seems to spread from there), right side chest pain. She didn't do any physical things to hurt the muscle... so what is it? Is is urgent?

This sounds more like gallbladder pain - e.g. gallstones. Did it start after eating?
Whatever it is, you need to see a health professional for a proper diagnosis.

Heart attacks or angina present in very similar ways! Signs and symptoms usually include pain in the centre (front) of the chest (e.g # feels like an elephant is sitting on the chest), sweating, pale skin apperance, nausea, clammy, shortness of breath, pain radiating into the left arm, jaw or neck, rapid irregular pulse. If the individual has any of these signs or symptoms it should be classed as a medical emergency and medical aid or assistance should be sought immediately!

Pain in the back and shoulder may be a number of things musculoskeletal in nature! Sometimes the posture or normal activities of daily living can cause pain or injury, especially in the elderly population. I would suggest that you get this person checked out by a doctor and if he or she then feels its related to the musculoskeletal system you should follow this up by seeing your friendly physiotherapist! it could be a heart attack and especially if you think so, go to the ER. heart iscemia (blockage) can radiate to the back.

But people can experience muscle tension and tightness that restricts blood flow and sqeeze nerves even if they dont hurt themselves. everyone experiences stress, this can cause this stuff. you can have a nasty cold and have back, chest, neck and shoulder pain. you can have gallstones. you can sleep wrong too.
case:
a woman, a new mom, walks into the clinic with severe stabbing central/left sided chest pain radiating down her left arm (classic heart attack signs). she had been to the emergency room prior thinking she was having a heart attack. blood tests returned negative. - it turns out that her pectoralis minor (a group of muscle in the chest under the pec. major) was in spasm and impinging the nerve. being a new mother, she had to carry the baby and always used her left side because she was right handed.

Gall bladder pain typically "refers" to the area you're describing (when pain is felt in the body in an area other than the area of pathology). There could also (not as likely) be pancreatic involvement. Has she eaten a fatty meal today? Has this occurred before? Even if the pain goes away, she needs to consult a physician.

That doesn't sound like a heart attack, but..

...it could be a blood flow problem, so it is still very important to go to a doctor. A clot can cause severe pain and isn't something to mess about with.

Heart attack is usually pain in centre of chest, down left arm, sometimes up into the next, hot and cold sweats. if shes in pain take her to hospital

Don't worry, it's probably nothing. If she loses consciousness, starts turning blue, or her heart actually stops, then you should probably call a medical professional.
