I am 25 and have had the same problem lately. Well, actually I've been that way for as long as I can remember, but lately its worse. I went to see a Dr because I thought I had ADD because of the problem with forgetting so much, and not being able to concentrate. The Dr talked to me for a while and realized that I had a anxiety disorder. He put me on medication, and although I do not feel 100% better, I noticed a change. My advice would be to do the same thing I did and go talk to your Dr about it. It might be nothing, but at least you will get some sort of answers.

I am notorous for missing people's names, recalling the wrong name when I am talking rapidly about them (like using my sister's name instead of my wife's) and for taking a long time to call up names I am sure I know, like the author of a book or an actor. Have been this way all my life. If it concerns you at your age, you might talk to a psychology department about any health problems that this indicates in young people and perhaps take some tests.
