You can't always tell. I was for a while until I realized I had a problem...no one knew. I'm a small person naturally.. The only real signs are if the person sleeps a lot, or is always tired, if you can tell they aren't getting any food, and if they get light headed really easily...and black out sometimes. But that is only in serious cases.

If you are around the person a lot, you will be able to tell if they are bulimic or anorexic if you see regular signs in the bathroom of excessive vomiting. Some use lots of laxatives.

Some of these folks are good at hiding their actions and chances are, the person doesn't even know they are sick.

Seek professional guidance by calling your local hospital or clinic as to how you might be able to get them in for help.

You simply can't tell by looking at someone. The only way you can tell is by their actions, and even then you can't be certain. If they refuse to eat anything because they think they are too fat when they are actually thin, then they are likely anorexic. Really, though, only a trained professional can diagnose anorexia. It isn't that simple. Being skinny does not make one an anorexic.

Anorexia--medically speaking means lack of appetite having nothing to do with phyisical appearance. It can occur temporarily because of an illness or chronically (long-term) because of a psychiatric/mental condition.

This site although not a true medical site--does have some good layman's terms for eating disorders and some reasonable advice.

A person is anorexic if they are less than 85% of their ideal body weight, and are engaging in some kind of restricting and/or binging/purging.

Anorexic people start looking unhealthy. Sometimes their hair falls out or their skin looks bad or their face looks all drawn. I don't know all the outward physcial appearance symptoms, but check something like webMd.com.

A BMI below 15 is considered under weight...below that is anorexia territory!
