You can do one of two things. The better thing to do is to call your former employer and get a copy of your W-2 sent to you, then file an amended return on form 1040X.

The other thing you can do is just to wait for the IRS to catch it - and they will, although it could be a year or so before they do. If you owe, you'll pay not only the tax you owe but also interest and penalties. If you have a refund coming due to that W-2 being added, you might or might not hear from them.

Call the company and have them send you W2 for to your new address. When you get it you will file an amended tax return that includes this income. Don't think the IRS doesn't know. They do and they will penalize you if you don't file an amended return as soon as possible.

You should contact the employer that you had the job with and ask for a duplicate W-2. With that you have to have a 1040X filled out so you can get an additional refund.

From what I recall you do not need to file for any income from any one source if the income for the year was under $600. (Which is also why you have to go to the local lottery office for any wins larger than $599.)
