You mentioned she does this "sometimes". Does she tell you when she need to go out most of the time and just have these accidents occassionally?
Perhaps the need to go to the bathroom comes on her so quickly and urgently sometimes that she doesn't have time to ask you to take her out. She knows she isn't supposed to go in the house, which is why if you're near her when she gets one of these urges she'll run into another room. She definitely is trying to hide it because she knows it's wrong, but maybe she can't help it if the urge is strong enough.
Maybe a vet could help you figure it out. If her need to go to the bathroom comes on so quickly and urgently she might have a medical issue.

She doesn't understand not to go in the house, she just thinks she's not supposed to go in front of you.
Do you go outside with her so you can reward her for doing her business there? If not, she won't understand what she's out there for.
Start keeping her tied to you for now when she's in the house. This way she can't sneak off and mess, and you're more likely to notice that she has to go. Go out with her so you can reward her for correct behavior.
Make sure the new habits are really ingrained before you start giving her a little bit of freedom in the house. Make her earn her house priveleges.

She does this to challenge your dominant behaviour. To re-enforce your dominance you need to catch her doing her business in the house. Once you catch her grab her by the scruff and shake her (Don't hurt her), push her to the floor and hold her there for a few seconds, much like a leader would do if she were in a pack. In a humans way of thinking..this might sound cruel...but in a dogs mind it's a sign of dominance. Once this in done take her to where you want her to go and when she is not watching...clean up the mess.
Your lucky you have a small dog. Mine being a Retriever has started this and I have to flip her into submission. It totally works. She hasn't tried going in the house for days.

If you scold a pup for housebreaking accidents, she will think it's wrong to go to the bathroom, so she'll hide from you to do it. That's why dogs should be housetrained with positive re-inforcement only. Ignore mistakes, reward good behavior.

your dog is trained but u now sometimes they forget so when he has accidents he gets scared that he might get in trouble or something so next time she does that just give her a firm no and give her a time outside so she learns not to do it

My dog does the same thing. It is almost like when the dog can't see you, she gets nervous. Also, she probably runs away and goes on the floor because she knows it is wrong.
