I've seen people put ribbons on the cake knife; I've also seen a small bouquet almost like a wrist corsage used, complete with ribbons and streamers in complimentary colors.

Whenever I decorate one, I make it with a very small cluster of flowers with ribbons and a pearl spray. I try to use one or two flowers used in the brides bouquet, of course matching the colors. I like to do it in artificial flowers so she has a memory of the colors of her wedding.

Michaels sells inexpensive cake knives & servers for weddings. You could also put ribbon around the handle by gluing it then braiding it around the handle. Add silk flowers around the handle.

Attach a flower to the handle, close to the blade, then tie a color coordinated ribbon around that. Use a flower that is in the bridal bouquet.
