Ummm ... has he seen a doctor? Simple liver function tests should be the first step.

See a Doctor, get blood tests!

If you want to add more information I can answer better.
What medication is he on?
What is his diagnosis?
Does he drink, do drugs, etc.?
Any other info you have ...

Wouldn't intestinal tuberculosis perhaps give you ascites, as ascites is simply a symptom not a disease. What medication are they giving him? How long has he been diagnosed with tuberculosis. When did the ascites start? Did it occur following an abdominal operation?

If the tuberculosis is controlled, it may stop and there are some cancers which cause ascites. I know a few friends and relatives who had this problem and in all their cases, cancer was established later.

I hope your brother will get well soon.

Sounds like his liver. If he is taking strong meds they could be ruining his liver. They should be checking his liver enzymes. He may have developed hepatitis or cirrohsis.
