You know my big problem is that I'm LAZY! I've got plenty of great ideas, I'm intelligent and capable in many respects but I'm lazy.

Now I fully expect people to say something like "get off your lazy *** and do something then" and, in truth, that would not be bad advice. What I really want, however, is to WANT to get off my lazy *** and do something. I need to find the kind of enthusiasm and drive such that I wouldn't need to push myself.

Am I just asking the impossible or has somebody got the answer? Please tell me!

I think you need to look at what really interests you, what are your passions, likes/dislikes, have you any hobbies or anything you would love to do before you die. Ask yourself what you would do in the next year if the "laziness" magically disappeared. If you try writing down all the ideas/ things to do however simple I bet you will find at least 1 you can achieve in the next 12 months, which if you actually do, may help you stop thinking of yourself as "lazy" and help break the cycle.
You say you have plenty of ideas, what about?, perhaps you need to look at why you have not followed through on these, perhaps your personality is more suited to having others around you to help you achieve these ideas, motivate you, set you targets etc. Some people do need more of a push than others, and sometimes breaking down a "task" into easier to manage parts can help to kick start motivation.
Setting a routine to do something rather than just sitting at home feeling bored/lazy may help you break bad habits.
Sometimes just getting up and doing something active even if it's just something simple like going for a a short walk can energise you for other things.
Finding a way of rewarding yourself when you do stop being "lazy" may also be a good idea.
Really it is up to you, become who you really want to be, you can do it
