Do I need any type of form to fill out and how much will it cost to change the name on the mini van? Who do I see about all of this?

The owner should be able to get a duplicate title from their state's DMV office.

M y knowledge is in Ca, so I'll try to keep this general and hopefully it'll help you.
----Is there a lien against the car?
Chances are that older cars are paid for but not always. If a title is not available you need to at least see the current registration card, it should show any possible lien holders and their info. If a lien is shown ask the person to provide a notarized lien satisfied. (They contact the prior lien holder for it not DMV)
----Is the vehicle more than 10 years old?
yes, all you should need is a written bill of sale (keep in mind that some states require it to be notarized.)
No, Check with your DMV on a federal approved odometer statement. Required on all vehicles under 10 years old.
----Ca charges $15 per transfer in addition to the registration fee.

tell the one that gave the gift to apply to their state for a lost title you can do so through the DMV
