You said your doctor gave you 2 notes......but you don't know which one it is---so you're asking what it would look like? LIE.

they usually use whatever paper they have handy in their office, and just write a more professional version of what you ask them to write and they sign their name and put the doctors office stamp on it with the doctor office address.
