Eres means "you are", so ?Que eres usted comida favorita? Would mean what are you you(formal) favorite food?
You should say "?Que es su comida favorita?" The verb (ser), is refferring to the word comida, not the usted.

No, because instead of que you would say cual (which), and eres is the you form, instead you should use the usted form (esta). Also, su means your. So, the final sentence will look like this:
