No telling. Take it to a local tattoo shop and ask them how much they'd charge you. I imagine that the price will be subjective to whomever renders the tattoo on your forearm.

It depends on where you go, and what size you want it. Ihave one like this on my arm. 3 inchs high, and wrapped around my arm. It cost me $250usd. Its awesome work. you get what you pay for. Just make sure you check out the persons work before hand.
