While it can refer to two people independently romantically linked with a third, it usually implies that each of the three people has some kind of relationship to the other two. The relationships can be friendships, romantic, familial (often siblings), or even pre-existing hatred between rivals.

It is 3 people, similar to the sides of a triangle. There is usually 2 of the 3 in love with the remaining one. (Think of a married couple and one spouse is cheating on the other) The person the spouse cheats with and the person he or she cheats on are the 2 people dealing with the third. This is a lame situation that goes on more than you would think. It is essentially quite hurtful and disrespectful for those that get their hearts broken. This is not to be confused with a "threesome" as that is a sexual situation. A love triangle deals with feelings of the heart. (of course with sex involved usually as well)
