In Singapore you are considered a willing donor unless you opt-out by filing a form with the government. If you wish to be kept alive mechanically even after brain death, you need to create a "living will" specifying your conditions for discontinuing treatment.

Unfortunately, cadaver transplants are very rare in Malaysia, due mostly to cultural misconceptions. If you are a citizen, consider joining the grass-roots movement toward life-saving education.

If you wish to be an organ donor, the most important thing you can do is tell your friends and family. The next thing is to carry something in writing expressing your wishes. In the US, most states allow you to designate this on your driver's license. In other countries, you can probably find out by contacting either a hospital that does transplant surgeries or an organ recovery agency.

It's a good idea to also express your wishes in a will, however a will is usually not read until some time after you have died, which is too late for most organs to be transplanted.

When a person is "near death", it is important for their next of kin to know that they wished to be a donor, so that the proper steps can be taken by hospital personnel to best preserve the organs for transplant. In the event that family is not available, something in writing that is kept with your identification documents (in a purse or wallet) will usually suffice.

Most states just require you to check a box when you go to renew your license. I think they'll imprint it on the license itself or give you an organ donor card.

Oh you're in Singapore? Ooops I'm stumped for an answer for you. Check with a local hospital perhaps. That's what I would do.

Yes, you have to have some sort of legal advanced directive if you wish to donate you organs. It may be as simple as making that election on your drivers license or more if you want your body to go to a specific place like Body Worlds for plasticisation
