There are many different situations which call for different things. The main thing is to be a good friend. If I had to say something I would tell them thank God you are safe.
If you believe in God...if not so sorry did not mean to offend you or anyone other answerer.

I just tell them that I heard that they had been through a rough time and that I hope things are looking up for them. I ask them if there is anything that I can do to help out and tell them that I will be praying for them. Some folks would rather not talk about the situation and if I get those kind of vibes, I just change the subject to something else.

You may not feel as I do, but I usually say "thank god the operation went well...how are you feeling today?
Car accident: I was scared for you, are you alright now?
Long journey: Welcome back, you made it back safe and sound, thanks be to God.
Illness: You beat it! I am still praying for your success, until you are 100%.
These seem to bring encouragement and support to everyone that I have talked to and they are sincere. The other person may not feel joyous, but they are glad to have you on their side when they are down.

I am so glad that you are doing better!
I'm glad to hear you came through your (accident, disease, journey....)
I'm so glad to see you! You appear to be doing pretty well!
You've been through a lot, I'm glad you're coping. Can I help?
Sometimes a big smile and a heartfelt hug say it all.

Just be honest and speak from the heart.
"I'm so happy you're getting better" or "happy you're safe" are both good.
Also, the person may or may not want to talk. If you keep it short, you'll find out whether they want to elaborate on the topic.
Good luck!

If you don't want to drag god into it, and emphasize the person's role in surviving, you could say, "Glad you made it through this thing."

i guess it depends on how close you are to them, if you really feel that you have to say something then just try to speak from the heart. some people say way to much when it isn't needed.
sometimes less is more

You could just say. " I heard about what happened and I'm so glad to see your doing well. I thought of you often."

Congradulate? You mean something like "I'm so glad you're doing well."
