My family & I are planning on traveling to Portugal sometime this year and we would like to bring our dog he's about 12lbs. Has anyone done this? What are the procedures/cost?

Contact the airlines for regulations, each alirline has their own regulations some allow dogs in the cabin under the seat other the dog must travel as cargo, if in cargo weather condition can cause last minute cancellation and your dog may not be allowed to board

Cats and dogs entering Portugal are subject to special regulations (EC) No. 998/2003 of the European Parliament and of the Council. For detailed information please contact DSCV - Direccao Geral Veterinaria: Largo da Academia Nacional de Belas Artes, 2 - 1249 -105 Lisboa, Portugal. Telephone number: +351 21 323 9500. Cats and dogs entering Madeira islands on direct flights from non-E.U. countries, are subject to obtaining prior authorisation from "Direccao Regional de Pecuaria de Madeira". Countries can have strict regulations for pets entering the country , according to this you don't want to arrive in Portugal on a weekend or holiday, because you won't clear customs with it

Domestic animals (birds, dogs, cats, etc.)

Authorization for importation is necessary and must be obtained before entry from the Direcção-Geral dos Serviços Pecuários in Lisbon, with the exception of those animals imported from the countries within the European Union. All animals must be accompanied by a veterinarian certificate issued in the country of origin. Those animals coming from countries where rabies exist must be accompanied by a rabies vaccination certificate with issuance within 12 months(for dogs)or 6 months (for cats), but not less than 21 days from date of travel. Please note, the certificate must indicate the date administered, type of vaccine, name of manufacturer, serial number of vaccine and the veterinarian's declaration with name, signature, and official stamp.

Upon departure of the animal the airline should advise the airport of destination in Portugal so that a veterinarian be present for inspection upon arrival of the animal. A fee is normally charged for this service.

You would have to speak with Portugal customs as well probably. You would probably need to have verification that your pet is not infected with anything.
