couldn't say for sure--it could be hereditary and harmless. but my sister had a large one all her childhood and adolescence, because her thyroid was in the wrong spot. then she got a cyst in it that made it even bigger. they had to remove it and now she has a normal size one. So without knowing your g/f personally, it could be a lot of things. I am sure she's very self-conscious about it, though, so be gentle if you do ask. Make it clear it's concern for/ interest in her, not idle curiousity or making fun of her. at least it sounds like she's learned to dress well to hide it.

Okay, it's one of two possibilities.
The first possibility is that she is in fact a man.
The second possibility is that she is a woman.
An adam's apple is more properly known as laryngeal prominence. Men and women have them but they are not very prominant in women and very prominent in men. Every now and then you may find a woman with a laryngeal prominence unusually protruding.
Since it is vital that you find out which possibility is true, I suggest you walk right up to her and ask:
"Are you a man?"
I'd like to see her response.
