What it means is that generally nothing in life is truly "free". There is always some sort of cost involved -- maybe not monetary, but somehow, someway you usually end up "paying" for something that may appear free. For example, if you win a car on a game show, yous till have to pay taxes, registration & licensing fees, etc. So it is not really free.

The lunch is not free of cost.
You or someone else will have to pay for it, either with money or some other form of trade-off.

In Latin it is "Quid Pro Quo."

In English it is "The difference between sex for money and sex for free is that sex for money usually costs a lot less."

There is always a price to pay for something. Maybe not in money, but something else even if it is only pride or self esteem.
