not really sure what your question is.

a home carer does not only do a bit of housework, they have lots of other duties that involve giving medication, changing dressings, lifting the sometimes invalid person in and out of bed and onto the toilet.

if that was what you meant i dont think you should belittle anyone that does a job that these peoples childrens should be doing

No, that is absolutely wrong! Home Care Assistants do a lot more than housework. There are occasions when they may be called on to dispense medication, change dressings, aid the client/patient in dressing, toileting, bathing, etc.

Home care workers do a lot more than clean. It kinda irrates me when one of the clients I have refers to me to others as the housekeeper. Housekeeprs dont help you take a shower, give you meds, keep an eye out for signs of deterioration, take vitals, changes adult diapers, clean you up when you have urinary or bowel incontinece. Also most of the agencies(including the one I work for) require CNA licensing just like a nursing home would. Its not all that hard of a work but there IS more involved than just house keeping although with a client who is unable to get around help with house cleaning is a necessity.
