I want to start 'To Collect Pray Request'.
Please guide the proper way for doing to collect Pray Request.
I want to help those person who is in problem.

1. Know the person, at least their first name to bring before God.
2. Know the need, the disease or problem, or thanksgiving to be addressed.
3. Pray for guidance, how to effectually offer prayer for the above.
4. Agree in Faith, Christ will be with you.
5. Listen, listen, listen, Allow the Holy Spirit to speak (especially through God's Word)
6. Give thanks and have faith.

First of all the best prayer request is normally the people close to you like your family and friends.
Then on the other hand you could be in prayer praying and then somebodies face will come in front of you, pray for this person.
I have already been praying and all of a sudden I prayed for an unknown person with a VW Beetle car and I don't even know such a person.
The Holy Spirit knows all and will reveal to you who needs prayer.
In the meantime, you can maybe at your home church get hold of the leader and ask him/her for some names of people who are in need.

Make a box labeled "Pray requests"
and have a pen and cards by it. people will fill out cards, and put requests in the box or before a prayer,ask for any prayer requests

Prayer is a waste of time. It is a way of feeling like you're helping someone, without actually having to help them.
One man with a hammer can do more good than a billion people praying.
Instead of praying, try actually going out there and helping people. That would be a lot more effective than "prayer requests".

Find people to pray for and then tell God "hey God, This person really could use a hand." Personally, I talk to God much in the same way I talk to anyone else. There are no rules, just Believe and then have FAITH! Try it and find out.

Join a church and then go to prayer. Usually, people will say their prayer requests there.

Yeah why go out and do something useful for people when you can simply mumble words and accomplish the same thing!
