he might have mild to severe brain damage. hes still lucky though.

I totally agree with Jennifer and her comment of "The biggest question would be were they able to get air to his brain during this time period with CPR?" This will make a big difference. If no CPR was administered for 10 minutes or longer (brain cells start to die within 10 minutes without oxygen), the outcome will more than likely be grim. It is hard to tell how much damage he has sustained until EEG's, and other tests are performed.

The biggest question would be were they able to get air to his brain during this time period with CPR? If not you are treading on iffy grounds. My 6 year old son stopped breathing for 25 minutes and when they brought him back he was in a coma and unresponsive to everything. After MRI's and EEG's it was found 70% of his brain was dead. He lost everything that made him HIM and would never recover. We had to remove him from life support.
Two rooms over a young boy who had drowned was without air for 30 minutes and was also in a coma. His MRI showed brain swelling but no brain death. After a few weeks he left with his parents.
Hypoxia is a fickle thing. Its like a tornado that destroys one house and leaves the neighbors untouched. I hope for your sisters sake he is spared.

Since brain cells start dying after being without oxygen after ten minutes, the damage could be severe. However, if he was being assisted in his breathing, i.e. CPR, then there is a chance that oxygenated blood was getting to his vital organs.
There have been stories of people who had drowned in cold water, and were down for over 30 minutes, but because of the temperature of the water, their body went into a hibernative state, allowing them to recover with few problems. I must caution you that cases such as this are very rare, but they do happen.

this is highly unlikely unless he was frozen. People don't survive not breathing for 45 minutes or if they do they are brain dead. Conventional medicine says that the brain begins to die after 4 to 5 minutes without oxygen.

That happened one time to a little baby that was left outside like in 20F weather. The baby's heart had "hibernated" and was not breathing or conscious for about the same time an hour. But the baby woke up and was fine that I know of. =<)

He has a hypoxic brain injury, 45min without oxygen is way too long for quality of life.

over 4 minutes with oxygen causes brain death in most instances...if they are in cold water it sometimes takes longer.
