The ways to know your tongue is infected:
an increase in pain
pain that doesn't go away in 2 days
unusual swelling
fever
pus

One of the main ways to avoid an infection is to see your dentist to receive an antibiotic to be taken before being pierced. Once piercing is done:
always rinse mouth before and after eating or drinking
avoid hot or spicy foods
avoid alcohol
avoid coffee
avoid smoking but if you must, rinse before and after
of course, follow any other guidelines given to you by the piercer

One of the main dangers of tongue piercing, other than infection, is chipped teeth. So, be very careful how much to swish your tongue around in your mouth until you get used to the piercing.

If you do have a complication, see you doctor immediately! Infection can cause the tongue to swell so much that it will block your airway leading to suffocation. Also, bacteria from the tongue can spread rapidly and, due to this, there have been deaths from toxic shock syndrome and blood poisoning.
