There are a number of causes, but the most likely is vestibular syndrome - this is an infection or impairment of the inner ear, causing problems with balance as you're seeing. It could also be neurological.
Very unlikely your cat has had a stroke. Apart from being rare in cats, they are usually fatal and would not leave your cat with just a head tilt.
Do get the cat to the vet. Apart from anything else, not being able to hold your head upright must be extremely uncomfortable. Cats do not really show pain, even if they have broken limbs - sometimes they even walk on these, they're rock hard! They can remain stoic throughout problems that would cripple humans, trust me on this. So get it sorted!

This does sound like a stroke. I had this occur with an older cat and the cat also seemed a bit confused. I'd take the cat for a vet visit asap.

Your cat could be hypoglycemic. My cat is diabetic and we were told to look out for behaviors like that. Take him to the vet a.s.a.p. If you can not get to a vet right away, give him some karo syrup and if you don't have that you can use maple syrup. Vets suggest a syringe full and then give the cat something to eat. From what you describe, it seems like he's got an ear infection. In any case, this is not normal behavior. Your cat should be seen by a vet.

We had that problem with a rabbit once. The vet said it was an ear infection (the inner ear affects balance).
