I went to a dinner party once and answered a set of questions before I ate, unaware of this, everyone was served different food with different utensils (i.e soup and fork, salad and bread sticks) as a result of how they answered the questionaire....Does anyone have the instructions/or questionaire on this type of dinner party? I have searched and searched...

We used to do something called a "silly supper" that was very similar to this. We made a menu up of silly words we got from a balderdash game. we included the untensils---so not only did they order a food item for each course but the untensil for eating it.
Before dinner each person filled out a order from the menu board we displayed. the servers then served whatever they ordered with whatever utensil. SOme got dessert first......some got entrees etc. It was the best time. we used to do this all the time. it made for a great evening. We also one time were raising money for a cause so we "sold" forks to those who needed them for 5 bucks. You get a hungry man with a plate of spagetti and a toothpick he WILL buy a fork.
