Have you ever been in a church? I have been going since I was a child and I have never heard anyone in the pulpit say 'the wrath of God is going to strike you down' I would love to find out what sort of church you are refering to.
However in answer to your question it means that if you are an evil person and do evil things then God will get his revenge by making you ill.

There's a funny scene in an episode of "Bulls.hit!" where Penn & Teller are running around with helmets that have lightning rods attached to them, and pointing at them yelling, "Right here, God! Right here!"
And of course, nothing happens. We know the Bible says that God "is a jealous God." If the Bible was right about him existing, he wouldn't let his creations taunt him like that.

For me it happens when I have dairy products without taking a lactase supplement.
It is also pretty brutal on those around me too.
