I've never heard of it. My 9 year-old has been flying unaccompanied since she was 5, at least twice a year. She's always had good experiences. More often than not, they've upgraded her to First Class, no matter what airline we've used, and taken very good care of her on the trip.

Probably never---they take exceptionally good care of kids.
I flew my son across the Pacific (14 hour flight) as an unaccompanied minor when he was 10. I was terrified, but the airline watched him all the time---first to board, last to get off, drove him around in a golf cart---special treatment the whole way, and my parents had to sign for him at the destination. He had a great time, and still has fond memories of the trip today (he's almost 20 now).
