Would you want to dispute it on all three credit agencies. You can find all three of them by searching for Experian, Equifax, TransUnion. Each of their official websites will have forms for you to fill out to complete a dispute. Don't expect it to go away immediately or at all, it can take up to months for a disputed item to leave your report.

Write a letter to the credit bureau and the company that's on your report. Make sure you have these letters sent certified mail so that you can have proof that they were received and you may want to look into getting the letter notarized and keep you a copy as well.
