There are a few theories on the Adam and Eve, Cain and Abel issue. If you read the account of creation, there is a part that says God created them male and female, and it isn't until the next part where He created Adam. The whole story could be thought to mean that God created mankind, then He created Adam.
God was the God of the Israelites only throughout the Old Testament. When Jesus went back to heaven He assigned the disciples to go and spread the gospel. With Paul came the first time that anyone outside of Israel was offered salvation. Any country that came into contact with the Israelites in the OT usually noticed that their God was powerful, but rarely do we see them coming in to serve Him. He was simply the God of the Israelites until the time of Paul. That is why there are some places that still haven't heard of Him.
But..... He can be seen in nature, so that is why most cultures have some sort of belief in God or in gods.

Adam and Eve were not brother and sister. They were created separately and the only common thing they shared was their humanity.
God created more people after Adam and Eve. Their sons (Cain and Abel) found wives to marry, and the women they married were not their sisters, so clearly there were more people besides the "first family."
About people in 3rd world countries, you are right that all people have knowledge of God and creation, but Christ wishes that all men receive the fullness of the Gospel.
That is why mission work is SO vitally important. Jesus told us "go preach to all nations the good news." The Catholic Church has always taken this command seriously and has sent missionaries to the furthest corners of the world.
For those people who for some reason do not receive the gospel and therefore do not receive Christ, we can only beg God's mercy. But we must never forget that Christ Himself said "unless a man be born of water and the spirit (baptized) He cannot enter the Kingdom of Heaven."
Because of these words, Catholics have always taken very seriously the sacrament of baptism, administering it to babies right after birth and to those who have been converted to the faith.

The levital laws against in breeding came about a thousand years after the story of adam and eve. The theory goes that as sin progressed and the human race became more corrupt, in-breeding became wrong.
Almost every culture has a history of a great flood story. It's very possible that over the course of thousands of years, in oral traditions, that people lost touch of the God they once knew.

We are so repulsed because it is not a common act in our society anymore. The same way we would be repulsed at any action that is very foreign to our culture.
Time and distance DO NOT make the heart grow founder look at Europe and their beliefs compared to their beliefs a hundred years ago. Almost pre Christian to post Christian.
