"Hole" in the heart usually means one of the walls of the heart has a defect. Think of the heart as a house with one way doors, and blood as people. Normally, the house has four rooms, and people are supposed to walk in an orderly fashion, from room 1 (right atria) to room 2 (right ventricle), then step outside for fresh air (lungs) then room 3 (left atria) and finally to room 4 (left ventricle). If there was a hole in one of the walls, people can then randomly wander into rooms out of sequence:
Instead of 1-->2-->3-->4,
they can go from 1--> 3 or 3-->2
When this happens in the heart and blood runs out of order, it means deoxygenated and oxygenated blood can be mixed- this can mean the body doesn't get enough blood, or the heart has to work harder than normal to supply the body...over time this can cause heart problems; depending on where the hole is, it can also cause lung problems.
If the hole is tiny and people can only poke their heads through or only a few people get through, it's not a big deal...if it is bigger, you need to patch the hole (surgery).
Often, doctors can tell you have a hole in the heart if they hear the turbulent blood flow; this sound is described as a "heart murmur."
If the doors between the rooms are damaged (doors = heart valves), the doors will sound different and this may also be heard and described as a "murmur" by doctors. If a person has valve disorder, past valve infection, or a fake valve put in, these valves are more susceptible to bacteria, so they get antibiotics prophylactically when possible blood comtamination is expected (ie during dental work is done!)

A heart murmor or mitrovalve prolapse. If it's small it just means you have to take antibiotics before seeing the dentist or any sort of surgeries. Not usually a big deal, I was diagnosed with one ages ago. But see the doctor and specialists for your own diagnosis, of course. :)
