If one leg is bigger than the other leg , he could have a blood clot. He should go to the ER ASAP and get a venous doppler to check for this. Also diabetics can be prone to getting MRSA infection in the leg...ask for a culture and sensitivity (to tell what germ and what antibiotic to use for it) if you suspect infection.

My father has a similar problem. He has diabetes, and he often gets an infection in his legs which causes them to turn bright red (almost maroon) and swell, to the point that his skin turns shiny and an old scar on his leg has burst open several times. He has been hospitalized for it at least twice, and is usually given antibiotics intravenously over a period of several days to get rid of it. It remains in his system, though, so he has relapses about once a year. If your friend's problem is like this, he needs to get in to a doctor right away, preferably a specialist. The longer he lets it go, the worse it will get.
