My mother in law has her blood pressure measured at 201/88 by a nurse [not GP]

She is 92 years old and in adequate health generally.

Is the systolic figure something to worry about and if so what might it indicate?

your mother-in-law is very old already, and isolated systolic hypertension are considered as "normal". this is because in the process of aging, the blood vessels becomes stiff and more resistant. so the heart needs to pump a lot harder to deliver the blood, thus, a higher systolic pressure.

a systolic pressure over 150 is always something to be concerned about, it can indicate fluid overload, restricted blood flow making the heart work very much harder than it should.
However, blood pressure changes alot through out the day and the concern is for a sustained elevation. She should check her blood pressure first thing in the morning before getting out of bed and once in the afternoon for about a week to see if the elevated pressure is consistant... if it is, she needs to see a doctor.

Her blood pressue with age and any anything over 150 at any is somethig to worry. There is a old phrase high pressure is a silent killer. Tell or come with here to see a doctor. There a new and improve medicines out there .

The systolic figure looks really high. Tell her to go to the Doctor and see if there's any hypertension involved. Do you have hypertension in your family? Do it just for your peace of mind and hers too.
