A "weak heart" is, by definition, cardiomyopathy. (Cardiomyopathy is, literally, heart muscle tissue disease.) Our hearts generally have quite a bit of excess capacity, so we are able to tolerate a certain amount of functional loss before even noticing. Also our hearts will tend to expand to "compensate" for the loss of function. However, this expansion can only go so far before the heart "decompensates" with leaking valves, electrical problems, or dyssynchrony.
At some point in this spectrum of compensation and decompensation, those with cardiomyopathy will be considered to be in "heart failure". I have not heard a good clinical definition. My wife's case, for example, is relatively mild, so it isn't clear if she is in "heart failure" or not. And it really doesn't matter. What matters is that you find the answers to all of you questions and get the help you need.
So I'll add a few links to resources that I think might be handy in your quest for knowledge. Please feel free to ask any further questions by sending me a private message or joining us at the Yahoo! cardiomyopathy group.
I hope this helps!

Failure of the heart muscles to eject adequate amount of blood to organs like kidneys, lung, liver etc is termed as a weak heart.
It also known as CCF (congestive heart failure) due to multiple causes.
Fluid is collected in the body i.e swelling of legs, in the lungs, and enlargement of liver as the right ventricle fails.
The kidneys fail to receive enough blood to filter out water and other toxins, hence generalised swelling of whole body, this occurs in extreme cases almost terminal stage of weak heart.
THIS WAS JUST A BRIEF SUMMARY FOR UNDERSTANDING HOWEVER IT A WIDE SUBJECT, HOPE IT HELPS YOU WITH SALIENT FEATURES.

Cardiomyopathy is a weakening of the heart muscle or a change in heart muscle structure. It is often associated with inadequate heart pumping or other heart function abnormalities.
Please see the web pages for more details on Cardiomyopathy.
