word by word meaning of what is your name, is, "aap ka naam kiya ha?"
but like in English, may I know your name? we have formal ways of asking some ones name.
Kiya main aap ka naam poochh sakta hoon? or Hazoor ka naam?

Rafta rafta means, slowly slowly, or with the passage of time.

"Rafta rafta wo meray hasti ka saman ho ga'ay" (slowly slowly he/she got entered into my innerself or become valuable to my existence)
I hope it helps.
