Lemon extract is made from the zest (the yellow part of the skin), usually using alcohol to pull out the flavor.

Lemon juice is what you get when you squeeze a lemon... it can be sterilized, pastorized and bottled, but same thing.
Lemon extract is made from the peel. I am sure you have noticed that citrus fruit peel contain essential oils - very aromatic (when you peel a fruit, your hands smell wonderful!!! because of those oils). Lemon extract is made with those, it is much less acidic than the juice, and more aromatic.

No you would add grated lemon rind not juice to a recipe if you wanted to replace lemon extract. Grate only the yellow part of the rind.
